Taxpayer's Declaration

I declare that all information I have provided in this form is true and correct to the best of my knowledge. I have disclosed all relevant income, deductions, and personal details required for the preparation of my income tax return.

I authorise my tax agent to collect and use my Tax File Number (TFN) for the purpose of preparing and lodging my income tax return. I understand that my TFN will be treated as confidential and disclosed only as required by law.

I acknowledge that my personal information, including my TFN and financial details, will be collected, used, and stored securely by my tax agent solely for the purpose of preparing and lodging my tax return. My information will only be disclosed as required by law or with my consent.

I authorise my tax agent to prepare and lodge my income tax return on my behalf, including electronic lodgment, and to communicate with the Australian Taxation Office (ATO) regarding my tax affairs as necessary.

I confirm that I have provided, or will provide, all supporting documentation necessary to substantiate my claims, including income statements, receipts, and records of deductions.

I understand that it is my responsibility to respond to any additional questions or requests for information from my tax agent or the ATO.
